**Job Description**:
**POSITION PURPOSE**:
Reporting to the Director of Care/Supervisor of Care, and under the guidance and direction of the Registered Nursing staff, is responsible for providing care, support, assistance, and supervision to residents/clients in the Health Care Centre; promoting independence in activities of daily living, including restorative care, and working in collaboration with members of the client care team.

**MAJOR RESPONSIBILITIES**
- Participates with other members of the Care Team in promoting the health, well being and independence of residents/clients.
- Performs basic care procedures which s/he has been prepared for through instruction and training by Registered Nursing staff.
- Promotes independence in Activities of Daily Living (ADL) by assisting with personal hygiene, dressing, transferring and toileting.
- Reports changes in residents’ physical, mental, and/or emotional condition to the Registered Nurse or designate.
- Immediately reports any problems or incidents involving residents and/or staff to the Registered Nurse in Charge.
- Receives verbal and/or written reports from outgoing shift members and documents incidents and conditions occurring on previous shift.
- Liaises and promotes positive relationships with the medical and facility staff, residents, families and visitors.
- May occasionally be required to respond to emergency situations outside of normal business hours, as related to the Departmental Emergency Plan.
- Performs other related duties as assigned.
- Replaces staff and travels to alternate Health Centre, as required/directed.

**QUALIFICATIONS**
- Completion of a Personal Support Worker certificate program or approved equivalent combination of education and experience.
- Up to one (1) year experience or practicum placement with resident care in a long-term care facility or other geriatric environment.
- Satisfactory Police Vulnerable Sector Checks (PVSC).
- Valid Ontario Class “G” driver’s license and reliable vehicle for use on corporate business.
- Good interpersonal and communication skills to work within a diverse community and ability to deal empathetically with residents, family members, etc., to contribute to their well-being.
- Ability to work effectively independently or as a part of a team.
- Ability to read, write and follow written and verbal instructions in English.
- Physical stamina and ability to lift in excess of 100 lbs with assistance; to lift and to carry equipment, supplies and patients; and to push and pull wheelchairs, wheeled beds or stretchers.
- Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration, and personal ownership.
