About the Opportunity

This is a designated position. The successful candidate must self-identify as African Nova Scotian or Black Person of African Descent in order to be considered for this role.

Reporting to the Health Services Manager, the Wellness Navigator supports citizens of the communities that the Community Health Team serves in accessing the services, programs, and/or resources to enable/foster health enhancing behaviors. The Navigator provides guidance and support for clients including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living. Is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives. Travel within the catchment area maybe required.

About You


• Baccalaureate degree preferably from Occupational Therapy, Recreational Therapy or Social Work (Bachelor of Social Work)

• Current registration with professional association

• Three years' recent related experience working in mental health, Primary Health Care setting/community-based team/community setting

• Experience with individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principle

• Experience working with people living with a chronic condition

• Preferred experience in partnership development, stakeholder collaboration, and community development

• Demonstrated understanding of population health, health promotion strategies, and behaviour change principles; in-depth knowledge of health authority and community based services

• Demonstrated person-centred/family-focused, diversity relevant approaches to care

• Well-developed interpersonal, teamwork, oral and written communication skills

• Demonstrates flexibility, adaptability and willingness to change

• Demonstrates innovation in program planning, implementation and evaluation, and quality improvement planning skills

• Computer skills (MS Office, email and internet)

• Annual CPR-AED certification and annual WHMIS training and certification

• Valid Nova Scotia Driver's License and access to a reliable vehicle

• Competencies in other languages an asset, French preferred

Compensation and Benefits

$36.58- $47.35 hourly
