Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care.We are looking for an Occupational Therapist (OT) to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life. This position works within a collaborative multidisciplinary team consisting of various healthcare professionals from the nursing and allied health professions. This team collaborates and conducts home or site visits to deliver client-centered services to individuals with a wide variety of physical, cognitive and mental health conditions, including complex health needs. As an OT in the Southwest Home care office, you will be providing case management as well as direct care, utilizing your strong skills in family-centered practice, coaching, assessing, organizing and communication. The Case Management duties include ongoing assessing and identifying the overall needs of the client, in collaboration with the family and other partners, and developing a care plan based on those needs. The care plan ensures appropriate services are provided, and the appropriate internal and external resources are accessed. Continuing Care OTs work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating and building relationships.Description: As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides. Required Qualifications: Completion of bachelor's degree in Occupational Therapy from an accredited university program. Registered with the Alberta College of Occupational Therapists (ACOT).Additional Required Qualifications: Bachelor of Science Degree or master’s degree in Occupational Therapy. Basic Cardiac Life Support- Healthcare Provider (BCLS HCP). Strong communication, assessment, conflict resolution and time management skills. Proven ability to provide exceptional clinical care within a continually changing healthcare environment. Physically able to perform all assigned tasks. The successful candidate will work independently in clients’ homes, which could include congregate settings, single family dwellings, condos and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.Preferred Qualifications: Familiarity with RAI, Connect Care, and case management principles. AADL Authorizer. Continuing Care experience. Connect Care experience. Palliative Care experience.
