We are seeking a registered Social Worker who thrives at the intersection of patient support, clinical coordination, and operational excellence.This is an office-based role, Monday to Friday. This role is ideal for a Social Worker who enjoys working at the systems and coordination level—supporting families through complex transitions, anticipating risk, and ensuring care teams are thoughtfully matched, prepared, and supported. You will have a meaningful influence on patient experience, staff readiness, and care quality without carrying a frontline caseload.  About the RoleThe Patient Liaison plays a critical role in ensuring that patients with complex needs and their families are matched with well-prepared, well-supported nursing teams. You will work closely with Ontario Health atHome (OH@H), internal clinical leaders, and families to coordinate care, prepare staff, and support quality and risk management processes.This role also supports workforce readiness and quality assurance initiatives, helping ensure that nurses are trained, confident, and aligned with each family’s unique needs.Key ResponsibilitiesPatient Services & Program CoordinationAct as a consistent point of contact for families navigating complex care needs, transitions, and service changes.Support family-centered planning, communication, and escalation for high-risk or high-complexity casesManage complex care nursing programs, including paediatric and adult clients, in partnership with Ontario Health atHome (OH@H).Build and maintain strong relationships with Care Coordinators and external stakeholdersSource, match, and prepare nurses for existing and prospective clientsCoordinate client-specific training, including SickKids modules, skills labs, and shadow shiftsAttend OH@H meetings and bi-weekly huddlesAssist with incident reviews, investigations, and formal reportingCoordinate supply ordering and inventory related to complex care programs Staff Preparation, Quality & ComplianceSupport onboarding and orientation of new nursing staffEnsure compliance with certifications, annual training, and documentation requirementsCoordinate ongoing education (e.g., GPA, skills refreshers, specialized training)Review and action operational reports (progress notes, documentation quality, attendance)Support performance management processes in collaboration with leadershipParticipate in Quality Assurance, Risk Management, and Health & Safety initiativesAdministrative & Team SupportSupport scheduling during peak periods or coverage gapsRespond to intake and service inquiry calls as neededContribute to the continuous improvement of workflows and patient experienceRequirementsBachelor of Social Work (BSW) or higher (MSW preferred)Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)3–5 years of experience in healthcare, hospital, home care, or community-based settingsStrong understanding of complex family systems and high-acuity care environments, including pediatrics and/or adult complex careExceptional organizational and time-management skillsExcellent interpersonal communication and professional judgmentComfort working in-person as part of a collaborative office-based teamWho You AreCompassionate, calm, and solutions-orientedHighly organized and able to manage competing prioritiesConfident working with families, clinicians, and system partnersProfessional, discreet, and thoughtful when handling sensitive situationsMotivated by meaningful work that improves patient and staff experiencesBenefitsCompetitive salary, with a performance-based bonus structure, and growth opportunitiesHealth and dental benefits with health care spending account and an RRSP match programMake a real impact on families with complex medical needsUse your clinical judgment and social work expertise in a systems-focused, high-impact roleBe a key connector between families, clinicians, and health system partnersWork with a supportive, close-knit, experienced leadership and clinical teamStable, office-based role with a strong connection to patient care
