The Dietitian I – Chronic Disease Management is a front-line professional position within Nutrition Services, reporting to the Manager of Nutrition Services, South. As a Dietitian I, you will be responsible for assessing, developing, implementing, and evaluating nutrition care plans for adult and pediatric clients within the Diabetes Specialty Clinic, Weight Management Program and Chronic Disease Management programs. You will also support the facilitation of nutrition sessions with the weight management series and other chronic disease group nutrition classes, virtually as well as in person. You will work with a multidisciplinary team with Alberta Health Living Program and Nutrition Services as part of the Diabetes Specialty Care, Weight Management, and other Chronic Disease Management teams.Description: As a Dietitian I, you may provide nutrition expertise to patients (clients, residents) in various AHS facilities and community settings to achieve nutrition and health outcomes. You may be responsible for nutrition assessment, development, implementation and evaluation of nutrition care plans. You may plan and provide public health nutrition services and education programs to promote healthy living and disease prevention for clients/groups in facilities or community settings. Nutrition education is provided to students and other healthcare professionals to support delivery of evidence based nutrition information. Required Qualifications: Bachelor of Science with a major in Foods and Nutrition. Completion of an internship program or practicum training program accredited by the Dietitians of Canada. Active or eligible for registration with the College of Dietitians of Alberta (CDA).Additional Required Qualifications: As required.Preferred Qualifications: Certified Diabetes Educator (CDE). Demonstrated effective group facilitation skills. Experience with Diabetes and Weight Management. Experience with nutrition counselling for behaviour change. Knowledge and experience with virtual health applications for patient care and group facilitation (Zoom, MS Teams, teleconference).
