Job DescriptionYou will be responsible for providing high-quality nursing care to patients in the community. You have clinical skills and extensive knowledge of wound, ostomy and continence care management for a wide range of patients with chronic diseases.Conduct comprehensive assessments and develop individualized care plans for patients with wound, ostomy, and continence concerns.Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of our teamEnsure that nursing care provided to our patients is based on current and best practices to achieve successful outcomesContinually assess, anticipate, identify and respond to new and changing needs of our patientsTravel between patients’ homes within a specific geographical regionQualificationsMust have completed NSWOC or International Interprofessional Wound Care Course (IIWCC)Currently registered and in good standing with the College of Nurses of Ontario (CNO)Minimum of two-year nursing experience (Community Nursing experience is ideal, but not required)Ability to work independently in managing your own caseloadProficiency in English, both verbal and written required (if you can speak additional languages, terrific)Valid Ontario driver's license and reliable vehicle with willingness to travelBasic proficiency in Word, Excel and ability to learn software#Peel_nurseAdditional InformationThis position is a current vacancySpectrum Health Care is thrilled to be named one of Canada’s Best Managed Companies.
