Position Summary The Clinical Pharmacist provides patient centered clinical pharmacy services including medication therapy management, therapeutic drug monitoring, drug information, drug use evaluation, adverse drug reaction reporting, medication counselling, medication incident reporting and follow-up. The Clinical Pharmacist will consult with prescribers, nurses, and other healthcare providers to develop and discuss therapeutic plans and assist in the development of order sets for individual disease conditions. Additionally, the Clinical Pharmacist performs drug distribution including processing orders by reviewing prescribed medication therapy; identifying safety concerns; contacting the prescriber and/or the healthcare team to resolve discrepancies or problems; verifying medication orders; documenting discussions or modifications to the medication therapy and dispensing medications.  Primary Responsibilities • Acts in accordance with the objectives, policies, procedures and guidelines of Orillia Soldiers' Memorial Hospital (OSMH), the Professional Standards of Practice, the Ontario College of Pharmacists, Accreditation Standards, the National Association of Pharmacy Regulatory Authorities (NAPRA), and the Federal and Provincial legislation relating to the practice of Hospital Pharmacy. • Consults with prescribers, nurses, and other healthcare providers to develop and discuss therapeutic plans appropriate for each patient based on chart review and assessment of clincial responses. Ensures appropriate documentation is maintained in the patient health record in accordanc with regulations, protocols, and practice standards. • Individualizes patient medication therapy by performing activities such as referencing standard literature/publications and patient demographic information, reviewing patient charts and laboratory test results, meeting with the patient and healthcare team to assess clinical responses of the patient to medication therapy and modifying or developing therapeutic plans to ensure that medication therapy is consistent with the patient's needs. • Provides patient medication counselling regarding medication therapy by reviewing reference literature, summarizing information and meeting with patients and their families to provide and explain information and plans, demonstrating procedures, techniques and use of equipment, and assessing patients/family's understanding of the procedures, techniques and equipment.  • Assist the healthcare team in developing and reviewing order sets for individual disease conditions by reviewing current literature, compiling and analyzing information relatingt to current therapy guidelines, providing suggestions and recommendations, and monitoring the effectiveness of current guidelines on patient healthcare outcomes. • Participates in unit/ward rounds, program team meetings, and provision of pharmacy services at the unit level to enhance patient care. • Acts as department resource person in the assigne area of responsibility. Supports both department and hospital team members by providing detailed medication information on issues related to medication therapy in those disease states which are treated within the program.  • Provides instruction and/or supervision to pharmacist students by preparing educational material, demonstrating procedures, supervising during patient care activities, and providing input into performance evaluations. • Participates in educational and teaching activities within the department, on the nursing units and throughout the hospital. • Participates in quality improvement activities through methods such as reviewing current practices and policies, performing quality assurance audits, identifying actual or potential problems, and recommending changes. • Performs medication distribution duties including interpreting medication orders and dispensing medications using a computerized medication information system in accordance with legal, professional, and departmental policies, procedures, and standards. • Reviews orders for therapeutic suitability by referring to standardized references, interpreting laboratory data, and/or reviewing patient specific demographics. • Communicates with prescribers, patients, and/or other healthcare team members for order clarification and problem resolution. Documents the outcome of relevant interactions with the prescriber in the patient's healthcare record. • In collaboration with pharmacy technicians, ensures that the medications are efficiently and accurately dispensed and/or compounded and labeled and that all necessary dispensing records are completed. • Participates in the collection of data for medication use evaluation projects and documents the required information. Assists in maintaining adequate inventories, notes shortages, requisitions medications and/or supplies, and monitors expiry data, reports procedural errors, and participates in quality improvement activities.  • Reviews cases of reported drug adverse reactions and completes necessary documentation. • Reviews medication incidents and is involved in follow-up and action plans to improve processes. • Participates in team member meetings, committees, and other programs as required; identifies opportunities to improve processes and/or procedures to enable safe and efficient operations and refers to manager and/or others. • Maintains records for narcotic and controlled medications, special access program medications, and departmental statistics in accordance with legal, professional, and departmental standards, policies, and procedures. • Provides continnum of care for patients through communication with community pharmacies and other external organizations, including Medication Reconciliation at admission and discharge. • Provides support to the Pharmacy & Therapeutics Committee by making recommendations on medication us and policy, submitting formulary requests and attend the meeting as required; participates in medication use evaluations. • Performs other duties as assigned. Qualifications • Bachelor of Science in Pharmacy or Pharm D from a Canadian University or equivalent according to the Ontario College of Pharmacists. • Current registration as a Part A Pharmacist with the Ontario College of Pharmacists. • Experience as a licensed pharmacist required. • Experience in an acute hospital setting preferred. • Knowledge of evidence-based clinical pharmacy practice. • Knowledge of medication use evaluation. • Knowledge of medication distribution processes in a hospital setting. • Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision. • Ability to communicate effectively both verbally and in writing. • Ability to establish and maintain effective working relationships with other health practitioners, as well as patients, family members, and the public. • Ability to organize, prioritize and plan work for efficiency and effectiveness. • Demonstrated commitment to continued professional and personal growth and development through Continuing Education Programs and Self Directed Learning Opportunities.
