The Position

is Canada's first inpatient intensive interdisciplinary pediatric pain service, supporting youth to regain function and return to usual activities, including school and recreation. Referrals come from outpatient specialized chronic pain clinics.

We are seeking a social worker to join our skilled and committed GUAG team on a temporary 18 month contract. We welcome applicants with availability between 0.6 FTE – 1.0 FTE (22.5 – 37.5 hours per week). In your application, please indicate your regular availability.

Responsibilities

Conducts strengths-based Social Work assessment and consultation for families of diverse backgrounds whose children may have a variety of medical diagnoses Assesses individual family dynamics and function, including strengths, areas of need and high-risk situations In collaboration with the interdisciplinary team, works directly with families to enhance parents' coping abilities; facilitates psychoeducational groups with both parents and teens ;provides supportive counseling during crisis situations Provides one-on-one psychotherapy interventions with families in addition to facilitating groups with parents and youth (teenagers) Supports and respects the client/family's rights to make decisions based on their own values, beliefs and experiences Initiates, implements, co-ordinates community referrals appropriate to client and family needs Bases practice on established Holland Bloorview policies, procedures, evidence and best practice Documents accurate workload statistics Attends and participates in program and professional staff meetings Supports inpatient units as required Adheres to Holland Bloorview's Documentation Policy Communicates in a timely and effective manner Contributes to the development of program/service activities Ensures the maintenance of the OCSWSSW Standards of Practice and Code of Ethics for Social Workers Identifies learning needs and establishes annual goals Remains current with professional literature and best practice guidelines Shares new knowledge and best practices with colleagues, staff, students and the community Participates in quality improvement initiatives, contributes to the academic agenda and supports the maintenance of a safe and healthy work environment and advances a culture of client/patient safety through work and daily practices

Qualifications

MSW Degree from an accredited University, Faculty of Social Work Current member of the Ontario College of Social Workers and Social Service Workers Current member of the Ontario Association of Social Workers preferred Previous job-related experience working with children and youth with mental health concerns essential Previous experience providing one-on-one psychotherapy services preferred Knowledge of and experience utilizing ACT, DBT, CBT and Mindfulness preferred Previous experience/knowledge of working with children/youth with chronic pain preferred Paediatric Social Work or health care experience preferred Experience working in an interdisciplinary setting preferred Works effectively as a member of the interdisciplinary team Demonstrates ability to manage and respond to crisis calmly and effectively Demonstrates ability to organize time and meet deadlines Demonstrates initiative and self direction
