Job DescriptionThe Behavioural Interventionist will support staff and informal caregivers, providing care to individuals with responsive behaviours that are unmanaged and are deemed as a barrier to being able to provide care. These behaviours may be associated with Alzheimer’s disease and related dementias, mental health, substance use, and other neurological conditions.Accountabilities:Conduct behavioural assessments as set out in Alzheimer Society Peel best practice guidelinesCollaborate with patient, caregivers and hospital staff to identify triggers to behavioursImplement, evaluate and document specific behavioural interventions in consultation with elder life specialistDevelops, monitors and evaluates the care plan recommendations by reviewing documentation, behavioural tracking and outcome measures to illustrate impactProvides patient specific education, training, modeling and coaching to hospital staff involved in patient careCreates appropriate referrals to additional BSO team members where needed to support the patient, caregiver and staffProvides support to ensure the successful transition of patients from the hospital to their next transition location through:The connection and collaboration with community services as needed to the patient and caregiversPre-transition planning which will include the patient and the caregiver; will include the adoption of a standardized risk assessment to identify the transition risksThe development of an individualized transitional care plan which will be shared with families and caregivers to support the transition and mitigate the identified transition riskProvide the necessary information to the patients and caregivers to support the transitionEnsures patients and caregivers are provided with timely and appropriate information and referral to the Alzheimer Society Peel and BSO servicesIdentifies staff and caregivers who would benefit from additional education and makes the appropriate referralsEncourages independenceImplements the necessary protocols to assist in the management of behaviours, facilitate success in managing behaviours and in providing careCoach staff in understanding and managing responsive behavioursDirect observation of patient during times of reported, responsive behaviours to identify triggers/unmet needs and recommend appropriate strategies to manage the behaviourParticipates in care conferences/patient huddles as requiredMaintains ongoing contact with medical health care professionalsInitiates and participates in ongoing collaboration with community partnersMaintains accurate and timely monthly statistics through the identified statistical reporting systemInitiates and participates in ongoing collaboration with community partnersMaintains accurate and timely monthly statistics through the identified statistical reporting systemPrepares and submits the required administrative documentation as requiredParticipates in administrative team meeting provides input and ideas for improving communication, services, client/caregiver support, as part of special projects, quality improvementQualificationsCommunity college diploma in a health care related field (Bachelor’s degree in a health care related field is preferred)Certificate in gerontology preferred or demonstrated evidence towards completion is preferredGently persuasive approach certifiedOne year recent experience working with the seniors and those with cognitive impairment in an acute care settingExperience with the prevention and management of geriatric syndromes such as delirium/BPSD preferredAttention to detail and expectation to meet documentation standardsExcellent communication (written and verbal) and interpersonal skillsDemonstrated ability to work as part of an inter-professional teamAbility to work independently: initiating, prioritizing and managing multiple tasksWill be required to travel between all Osler sitesMust demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation and CollaborationAdditional InformationHours: Currently Days, 8 hour shifts, including weekends and statutory holidays (subject to change in accordance with operational requirements)Internal application deadline date: January 12, 2026#LI-GG1 #TFTOsler values inclusivity and diversity in the workplace.
