This position understands the importance of quality and safety and requires a high degree of attention to detail and excellent time management skills. The position will be located within the Erie St. Clair Region. What will you do?The Nurse Practitioner will adhere to the effective delivery of Nurse Practitioner services for the Palliative Care Consultation Team’s patients, and community patients unable to access care through a family physician, clinic, FHT, CHC, etc. upon referral from Ontario Health atHome and in accordance with professional college standards, Ministry Guidelines and ESC policies. The objective of the services delivered by the Nurse Practitioner is to diagnose, treat, evaluate, consult and manage complex pain and symptom and associated chronic disease management issues during the patient’s palliative trajectory, in order to delay/prevent institutionalization and facilitate reintegration into the community. The Nurse Practitioner, as a member of the Palliative Care Consultation Team (PCCT), will work collaboratively with Hospice and be responsible for reviewing and completing assessments to assist in providing recommendations and consultative guidance to support challenging palliative symptom management and diagnoses. Travel for this position is required between all sites of Ontario Health atHome and there is the potential that the Nurse Practitioner may work out of the Hospice sites.What must you have?Registered Nurse, Nurse Practitioner (Current registration with the College of Nurses)Working knowledge of community resources and roles of health care professionalsGood knowledge of ESC priorities, policies, practices and service standardsGood knowledge and understanding of legislation governing health care activitiesKnowledge of direct care/care coordination models used in community health care organizationsSpecialized education required in palliative pain and symptom management including a minimum of Fundamentals and CAPCE. LEAP certification will be considered an asset. We have a mandatory COVID-19 vaccination policy.
