POSITION SUMMARY The Community Mental Health Worker-Shared Care functions as a member of the MyHealthTeam, working collaboratively with team members to plan, deliver and continuously improve client centered primary care services. Guided by the principles of MyHealthTeam, the incumbent’s primary focus is the provision of mental health clinical counselling interventions and promoting strategies that focus on optimal psychological health, disease prevention and promotion of overall health with individuals, families and communities, as well as facilitating service coordination and liaison with health care partners as required. Services are provided within a framework of advanced clinical practice, including consultation, assessment and therapeutic interventions. The incumbent plays a lead role in initiating and fostering collaboration between primary care providers and the consulting psychiatrist and other health care and community service providers within the MyHealthTeam. RESPONSIBILITIES: Overview: Depression/Anxiety/Adjustment Brief Treatment: Completes assessment of persons who are eligible or in process of being determined eligible for Shared Care Mental Health Services . Facilitates the development of a therapeutic relationship upon referral from the primary care Provider. Contracts with the client and obtains informed consent as indicated. Families and significant supports will be included in this process as agreed through out process of client consent. Formulates, implements and intervention and treatment plan in accordance with recognized best practice for individuals experiencing depressive, anxiety and adjustment disorders, utilizing evidenced based screening and monitoring tools. Tracks and monitors client’s pharmacotherapy and psychological status between appointments. Provides information, education, and support to clients and their families related to diagnosed psychiatric disorder and relevant treatment options. Works in close collaboration with referring primary care providers through referral, assessment, and treatment process. Evaluates the effectiveness of the treatment plan and adjusts as required in consultation with the referring primary care provider and consulting psychiatrist where appropriate. Refers clients to appropriate services as indicated. Consultation & Triage of other complex mental health presentations: Provides brief treatment intervention to triage, assess and make referral recommendations for individuals presenting with mental health issues other then depression and anxiety and adjustment disorder for whom it is unclear what the best treatment options may be. Refers clients to appropriate services as indicated. Crisis management: Crisis management and intervention as indicated. Assists the client in identifying appropriate crisis supports. provides support/service to regional trauma response teams as necessary. Facilitates and participates in mental health promotion: Promotes the mission and goals of the MyHealthTeam and mental health program by networking with MyHealthTeam partners and other programs and organizations. Participates in mental health promotion activities, which focus on decreasing stigma and barriers to individuals with mental illness, increasing mental wellness. Provides mental health education, training and consultation: Provides education and consultation for primary care providers. Identifies or assists in the identification of mental health needs in the community. Provides formal and informal education on relevant mental health subjects to the general public and community agencies. Participates in training, teaching and preceptorship of students. Administrative requirements and professional development: Participates in meetings and committee work as negotiated with supervisor. Consults and meets with supervisor on a regular basis. Participates in a matrix reporting framework. Utilizes peer supervision on an as needed basis. Maintains documentation as required by MyHealthTeam, regional policies and professional standards. Completes and submits required statistical information as required by MyHealthTeam and regional mental health program. Maintains professional skills by reading, attending meetings, conferences and training sessions. Schedule may include evenings and weekends as required. Travel included within PMH is an expectation of the position. Other duties as assigned.
