Position SummaryTThe OpportunityNorthern Health is seeking four Nurse Practitioners to join the innovative Hospital at Home program in Prince George. This full-time role involves providing acute care to adult patients in their homes, functioning as a virtual inpatient ward. Patients typically remain in the program for 7–10 days and include stable medical and post-operative surgical cases.Nurse Practitioners will act as the Most Responsible Provider (MRP) for their patient population and work as part of an interprofessional team that includes nursing, pharmacy, rehabilitation, and other allied health professionals. The schedule will evolve as the team is established, and mentorship will be provided for candidates who require additional support transitioning into the acute care setting.Key Responsibilities•Provide acute care to adult patients in their homes as part of a virtual inpatient ward.• Act as MRP for assigned patients, coordinating care and clinical decision-making.• Collaborate with an interprofessional team including nursing, pharmacy, rehab, and others.• Use PowerChart Electronic Medical Record (EMR) for documentation and care coordination.• Participate in rotating 12-hour shifts to ensure continuous coverage.• Engage with real-time virtual support for urgent and emergent cases.• Contribute to the development and evolution of the Hospital at Home model.About the ProgramThe Hospital at Home program is a new model of care that allows patients to receive hospital-level treatment in the comfort of their own homes. It supports early discharge and avoids hospital admission for eligible patients, improving outcomes and patient satisfaction. Prince George is known for its welcoming community, affordable living, and strong support for healthcare professionals.Salary RangeNew Graduate NP - $145,739 1 Year Experience - $153,026 2 Years Experience - $160,313Incentives & Support• Competitive compensation and benefits.• Mentorship and training available for acute care skill development.• Opportunity to be part of an innovative care model improving patient outcomes.• Professional development funds available based on a learning plan after 6 months of work. • Comprehensive orientation and ongoing clinical support.QualficationsEducation and Experience- Completion of BCCNM approved Master’s degree level nurse practitioner program.- Eligible for registration as a Nurse Practitioner with British Columbia College of Nurses and Midwives (BCCNM).Knowledge, Skills and Abilities- Ability to communicate and collaborate with patients/clients/residents and families about health findings, diagnosis, treatment, self-care and prognosis.- Ability to collaborate, consult with and formally refer patients/clients/residents to physicians and other health professionals when appropriate.- Ability to critically assess and evaluate health research literature to determine best practices; ability to introduce education and evidence-based research.- Ability to assess and recognize population health trends; ability to plan and implement strategies for population-based prevention and health promotion.- Ability to implement and evaluate planned change.- Ability to supervise others and evaluate the care they deliver.- Ability to lead a team and work within a team.- Ability to self-direct, interact, and adapt effectively with other professionals in complex, dynamic situations.- Ability to transfer knowledge, teach, coach and mentor others.- Ability to identify and respond appropriately to legal and ethical issues that may arise in patient/resident/client care.- Ability to self-assess performance and assume responsibility and accountability for own professional development, educational or consultative assistance when appropriate.
