The care provided in each Home includes specialized, restorative, supportive and palliative care for persons with dementia, disabilities and health problems who cannot live independently in their homes, and whose needs cannot be met in the community. You act as a lead quality improvement resource in the assigned LTC home by coordinating the implementation and evaluation of LTC home nursing programs in addition to supporting nursing staff with resident care plans and interventions. You also act as a resource to the LTC home for response to the Ministry of Long Term Care compliance issues and high-risk situations, and are accountable for monitoring the quality of resident care within the assigned LTC home. In addition, you provide coaching and mentoring to staff to ensure compliance with applicable standards and guidelines established for nursing programs, and implement and monitor clinical nursing practices and procedures to ensure the clinical standards are consistently met within the LTC home for quality resident care and better health-system outcomes, including lower hospital admissions and fewer emergency department visits.  You assist the unit with operational planning, conduct program evaluations, manage quality improvement projects, advise on clinical practice, including risk management strategies, conduct and remain current on latest research, support ongoing professional development of staff and recommended service enhancements, incorporation of best practices, and continuous improvement strategies. The Quality Care Nurse participates on committees with community partners to plan, organize and strategize on issues related to access to service, service delivery models, and enhancements/improvements to LTC programs. EDUCATION AND EXPERIENCE Completion of university degree in Nursing (BScN) Minimum of 2 to 3 years of related experience, preferably in a geriatric-gerontology or long term care home environment, including knowledge of best practices, performance measurement and training/coaching and mentoring of staff. CERTIFICATIONS AND LICENCES Registered Nurse (RN) current with College of Nurses of OntarioKNOWLEDGE Must have advanced knowledge and expertise in nursing practice  Research and analysis techniques/methodology In depth knowledge of the Fixing LTC Act 2021 and associated regulations Performance evaluation and benchmarking techniques Policy and procedures development and implementation Program design, development, implementation and evaluation Best practice approaches related to LTC nursing Assessment and intervention strategies Project management and process mapping Quality improvement methodology, or be willing to learn Knowledge of applicable health and safety legislation, including the rights and duties of workers COMPETENCIES, SKILLS AND ABILITIES Must be able to demonstrate and share their expertise through training, modelling, mentoring and policy development, and monitoring Must be able to communicate well with various professional and non-professional staff for outcomes to be achieved Conduct research, best-practice and operational reviews towards recommending service enhancements Manage a variety of multi-faceted projects using project management techniques/skills Effective presentation delivery and facilitator skills for various work/staff/management groups Write reports for formal presentation to senior management Establish effective working relationships with various different levels of staff within the branch, department, City and community Provide consulting services related to analysis, benchmarking, performance assessment and evaluation  Ability to understand the impact of priorities on other areas of the organization Effective client service focus in working with management and staff dealing with complex and sensitive matters Organized, efficient and productive in carrying out duties and responsibilities Capable of managing multiple tasks and priorities under tight deadlines and changing demands Contributes to overall teamwork, fosters strong working relationships with senior management, branch management, staff and peers Creates an overall positive effect on group morale Initiates innovative and results-oriented approach to service delivery strategies and problem solving Flexible  Self-directed WHAT YOU NEED TO KNOW Language Requirement: Designated Day 1 Ready – immediate requirement for language proficiency: French: oral, reading, writing English: oral, reading, writing Candidates must meet language requirement for position upon hire.
