Generally, patients transferred from other HHS sites to the SHF are waiting for: Home with Home Care supports, Transitional Care, Long-Term Care or require time to convalesce after an acute care admission. The SHF team is critical to achieving HHS’ vision of “Best Care for All” – supporting HHS patients to receive the right care in the right placePosition SummaryAs a member of the Satellite Health Facility, the candidate will provide service both as a case manager and physiotherapist to meet the client's needs. He/she will work closely with a physiotherapy assistant. The Physiotherapist participates in training, education and research activities to increase knowledge and to develop clinical, analytical and technical skills.Schedule Work HoursMonday to Friday; daysQualifications1. Current registration with the College of Physiotherapists of Ontario2. Experience in educating patients/families and in case management3. Demonstrated excellence in communication, interpersonal, team work and problem solving skills4. Knowledge of community resources5. Evidence of client centered approach to care6.
