The ideal candidate would be a motivated, self-starting, and proactive individual, committed to lifelong learning and excellent service delivery in the PEOL program. Are you someone who is passionate about PEOL, who has a drive to continually grow and achieve team and system- oriented goals? This is an NP palliative consultant role on the Palliative Community Consult Team (PCCT). You will work alongside a specialized team of palliative physicians, one other NP, and palliative nurse consultants to address referrals related to PEOL symptoms that are difficult to manage, complex family issues related to patients’ illnesses, disease comprehension discussions, complex GCD and ACP discussions, and Type C Continuing Care Home (hospice) assessments. You will work with system collaborators by assessing patients in their private residence, those residing in Type A/B/C Continuing Care Homes, and those as in-patients in Zone suburban/rural community hospitals. This work involves working with clinical partners in care: primary care providers, the Cross Cancer Institute, home care teams, Continuing Care attending teams, and acute care teams. You will work in an office setting, as well, will travel within established geographical networks of care in the Edmonton Zone. Your day-to-day work changes based on the location of the patients.Description: As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. PLEASE NOTE: This position is unionized and represented by the Alberta Union of Nurse Practitioners (AUNP). For more information on this union, please contact AUNP: Required Qualifications: Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure.
