*Qualifications:*
1. Previous employment in a health care/retirement or LTC setting.
2. Demonstrate an understanding of /empathy for the needs of seniors.
3. Demonstrate an understanding of mental health, addiction and dementia.
4. Good physical and mental health and free from communicable disease.
5. Certified in CPR and First Aid HCP level
6. Good understanding of the nursing process, critical thinking, problem solving and accountability.
7. Good written and verbal communication skills
8. Willingness to update and keep current their knowledge of changing regulations, practices and procedures.
9. CNO Licence in good standing.
10. Vulnerable Sector Police Clearance
11. TB test
12. ORCA training must be done prior to starting on the schedule
*Physical Demands:*
1. Standing for long periods of time
2. Walking short distance and up/down stairs
3. Typing and/or use of a computer keyboard proficiently
4. Pulling/pushing/lifting to open and/or relocate binders, carts, etc. up to 10lbs
5. Bending, kneeling, providing walking support for residents in the provision of resident care where required.
6. Dexterity required to open pouches/blister packs, insert test strips into glucometers, etc.
7. Reading/seeing documentation both in paper format and on computer screen
8. Hearing differences in phone ring tones and/or in fire system horns that alerts for emergency response purposes.
9. Speaking/communicating with individuals
*Job Description: Medication Administration*
1. The Nurse is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner by law and in accordance with prevailing practices as set out by the pharmacy.
2. Reviews, reads, documents and communicates pertinent information about residents
3. Receives medication updates
4. Administers/assists/observes medications and treatments for each resident using the Medication Administration Records (MARs)
5. Medications are passed according to times as directed by residents and/or physicians orders
6. Documents and initials as medications are given; appropriate documentation is completed for refused or missed doses.
7. Maintains confidentiality of all resident information including resident medication among other residents
*Additional Qualifications*
1. Successful completion of initial and annual medication administration training and signed off by the DOC
2. Notifies the Lead/Assistant lead nurse of any resident concerns
3. Restocks medication cart after all medication passes
4. Checks all medications prior to administration using the “8 rights” for medication administration
5. Provides PRN (“as needed”) medications as requested by the resident and as per the physician’s order
6. Counts all narcotics with another UCP/PSW or Nurse at the beginning and end of every shift. This includes PRNs in lock box at rest home and Nurses office.
7. Refills resident medications as required and as per the prescription refill process
*Job Description: Resident Care*
1. Notifies Lead/Assistant Lead Nurse of any resident and/or family concerns
2 Attends and actively participates in daily communication meetings
3. Provides feedback in the assessment of every resident and in the development of the Plan of Care along with Lead/Assistant lead nurse and family/POA/SDM
4. Answer all call bells when necessary
5. Observe and report any resident changes in condition
6. Complete nursing documentation if applicable as per residence policies and guidelines
7. Deals courteously with staff, residents, resident families and visitors.
8. Helps assist DOC/Lead/Assistant Lead nurse with doctor days, faxing medication changes, referrals etc. and communicate Dr. report in resident care plan
9. Helps residents maintain independence, promotes, dignity and physical safety of each resident.
10. Assist with monthly Blood Pressure and Weights as assigned.
11. Assist with monthly, q3weeks and weekly injections as per doctors’ orders. Assist with yearly vaccinations.
12. Provide emergency first aid/nursing care until appropriate medical care is available.
13. Call Family members/POA/SDM to notify of any falls, injuries, being sent to hospital etc.
14. Assist with resident admissions and discharges when required
13. Assist with any other duties as requested by DOC, Director, Administrator, or Human Resources.
*Job Description: Risk Management*
1. Partners with community team, to ensure community is in compliance with Occupational Health and Safety requirements and promotion of Risk Management programs and/or policies; adherence to safety rules and regulations.
2. Practices safety procedures at all times including Personal Protective Equipment (PPE), Fire safety procedures including extinguisher (as applicable), SDSs (Safety Data Sheets) and lockout tagout procedures
3. Reports all accidents/incidents immediately
4. reports all unsafe and hazardous conditions/equipment immediately
5. Complies with all infection control techniques, placement of biohazard containers and removal techniques, procedures and policies
6. Participates in Fire Safety and Mock disaster procedures
*Job Description: General*
1. Perform all other job duties as assigned
2. Refer and adhere to all policies and procedures as they are in effect
3. Ensure all resident charts are up to date and organized.
4. Have open communication with Doctors, HWS, RHRA, residents and families
5. Be punctual to your shift in order to receive shift report from previous shift.
6. Maintain compliance with RHRA by ensuring all Orca training is done by January 31.
7. Yearly continence product evaluation with resident and family
