We are looking for RNs and RPNs who desire great flexibility in their schedules.

This is a Full-Time Monday to Friday position operating during regular school hours in the Dundalk area.

This position is working for CarePartners supporting a patient in a school setting.

What The Role Involves:
Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
Working with patients (both in-person and virtually) to provide exceptional nursing services
Providing services in a variety of settings which may include in-home, clinic, retirement home or school
Communicating and liaising with patients, family members and others within the interdisciplinary care team
Performing other clinical duties as required
What You Bring:
Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
Current College of Nurses of Ontario (CNO) registration in good standing
A strong desire and commitment to making a difference in the lives of our patients
Exceptional interpersonal skills
A valid driver’s license and vehicle as regular travel is required
Excellent English verbal and written communication skills
CPR Certification
Current immunization record including 2 step TB test
To protect our patients, we require a current vulnerable sector check and a clear background check
