Job Title: Home Care Specialist, Integrated Care Solutions

We are seeking a highly skilled and experienced Home Care Specialist to lead our team of Personal Support Workers/Home Support Workers in providing exceptional care to our clients. The successful candidate will be autonomous, comfortable working independently, and able to thrive in a dynamic environment.

Responsibilities:
  • Oversee service provider personnel and ensure compliance with policies on client safety, risk reduction, and staff safety
  • Audit and analyze care plans for clients and make recommendations for improvement
  • Develop and implement clinical care pathways for clients
  • Provide education and training to staff members on new procedures and protocols
Requirements:
  • Bachelor's degree in Nursing from an accredited institution
  • Active registration with the College of Nurses of Ontario
  • Minimum 5 years of clinical nursing experience, including 2 years in a supervisory or leadership role
