You will support community clients through the Acute Care Transitions Program. The role emphasizes holistic care planning, professional autonomy, and collaboration with interdisciplinary teams. Services address physical, cognitive, social, emotional, and spiritual client goals.
Key Responsibilities:
- Provide client-centered care in community settings
- Support clients transitioning from hospital to home
- Participate in joint care planning
- Address physical, cognitive, social, emotional, and spiritual needs
- Deliver accountable care and services to assigned clients
Requirements:
- Valid Personal Support Worker certificate, RN, or RPN
- Valid driver’s license and vehicle for daily work
- Current Standard CPR level C certificate
- Current vulnerable sector check
- Availability for days, evenings, and every other weekend
