Job Summary:
Care Coordination is a vital component of the New Brunswick Extra Mural Program. The Liaison Nurse plays a crucial role in ensuring seamless transitions between hospital, community, and home care settings. This position requires a skilled nurse to coordinate care planning with patients, caregivers, and healthcare teams, facilitating efficient and safe transitions.

Main Responsibilities:
1. Coordinate EMP care planning with patients, caregivers, and healthcare teams to facilitate transitions between hospital, community, and home care settings.
2. Identify current EMP patients who are admitted to hospital and provide written or verbal information to the hospital regarding these patients. Dissemination of EMP Transfer of Care information is essential in the continuity of patient care across the continuum.
3. Provide EMP Units with updates on hospitalized EMP patients to help determine appropriateness of discharge from EMP services.

Required Qualifications:
1. Active registration with the Nurses Association of New Brunswick.
2. Minimum of three (3) years clinical registered nursing experience in home healthcare and experience working in acute care, within the last five (5) years.
3. Knowledge of the target population of Extra-Mural Services and available community services.
