We are seeking a skilled Primary Health Care Nurse to join our team at Interior Health Authority. As a casual employee, you will work autonomously and provide comprehensive nursing care to clients in a primary health care setting.
Key Responsibilities- Assess, plan, implement, and evaluate care delivery for clients in a primary health care setting.
- Develop and provide formal education to clients, families, and other health professionals.
- Collaborate with an inter-professional care team to conduct integrated care planning and provide ongoing monitoring and self-management support.
- Work in a community or patient home setting, providing care to adults, seniors, and pediatrics, as well as those with multiple chronic conditions.
- Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
- Advance clinical skills training in the assigned practice area.
- Three years recent related clinical nursing experience in the assigned practice area.
- Current valid BC Driver's License.
