Job Summary:

Pro Vita Care Management is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a key member of our care team, you will play a vital role in delivering high-quality care to our residents.

Key Responsibilities:

  • Observe, monitor, and document residents' physical and psychological conditions
  • Provide and assist with personal care to residents as required
  • Ensure adherence to infection control practices
  • Assess resident health status, including recognizing deviations from mental health baseline and differentiating between dementia and delirium

Requirements:

  • Graduation from a recognized Practical Nursing program
  • Occupational First Aide Level 1 certification
  • Satisfactory proof of COVID-19 vaccinations
  • Effective communication skills in English
