The role involves assessing, planning, implementing, and evaluating individualized care plans to meet the physical and emotional needs of residents. You will work collaboratively with other members of the multidisciplinary team to develop effective care strategies.
Your responsibilities will include:
- Resident Assessment: Conduct thorough assessments to identify residents' needs and develop personalized care plans.
- Communication: Maintain open communication with relatives regarding residents' care and condition.
- Team Collaboration: Liaise with attending physicians, psychiatrists, consultants, social workers, clergy, relatives, or responsible persons to ensure seamless resident care.
- Meds Administration: Assist with administering medications as required, adhering to established protocols.
- Workplace Maintenance: Ensure work and storage areas are clean, safe, and sanitary.
This position requires a high standard of empathy, understanding of seniors' needs, and effective interpersonal relationship skills. Experience as an RPN registered with the College of Nurses is mandatory, with memory/dementia care experience preferred.
We offer a competitive salary of $**45.00 - 50.00 per hour**, commensurate with experience, along with opportunities for growth and development in a dynamic healthcare environment. In addition to your hourly rate, you can expect a total annual compensation of approximately **$93,600 - 104,000** based on 60 hours bi-weekly.
