We are seeking a highly skilled and experienced Registered Nurse to join our team as a Care Team Lead - Integrated Care Solutions based in Toronto.
This is a full-time permanent role with competitive total compensation package, group benefits, generous vacation package, and in-depth training and orientation to set you up for success.
As a member of our team, you will oversee service provider personnel and ensure compliance with policies on client safety, risk reduction, and staff safety. You will also be responsible for assessing, planning, collaborating with clients, families, and other healthcare team members to establish and update care plans, and implementing and evaluating client care with every client encounter.
Key responsibilities include:
- Oversight of service provider personnel to ensure adherence to policies and procedures.
- Ensuring compliance with policies on client safety, risk reduction, and staff safety.
- Assessing, planning, collaborating with clients, families, and other healthcare team members to establish and update care plans.
- Implementing and evaluating client care with every client encounter.
Our ideal candidate has:
- An undergraduate degree in Nursing from a recognized accredited school of nursing.
- Active registration and good standing with the College of Nurses of Ontario, as a Registered Nurse.
- A minimum of 5 years of clinical nursing experience, including 2 years in a supervisory or leadership role.
- Knowledge of home care delivery, gerontology, and integrated care models.
- Highly organized with ability to multitask conflicting priorities.
- Demonstrated leadership, clinical, and communication skills.
We offer a competitive salary range of $120,000 - $140,000 per year, depending on experience, along with a comprehensive benefits package and opportunities for career advancement.
