The Indigenous Patient Navigator position is an exciting opportunity for the right candidate to join our team at Penticton Health Centre in British Columbia, located on the traditional territory of the syilx nation.

About the Role

This casual opportunity offers a chance to work with a dynamic and collaborative team, providing cultural expertise and commitment to patient care. As an Indigenous Patient Navigator, you will work as part of the Interdisciplinary Care Team, acting as a resource for both Indigenous clients and healthcare providers to ensure culturally safe and client-centered care.

Your key responsibilities will include:

  • Providing consultative and navigational services to health professionals, communities, and clients to understand and identify client needs by supporting cultural interpretation and/or health literacy translation services and by supporting culturally-competent practice (e.g., access, system barriers, and navigation challenges).
  • Supporting connection to community services such as Indigenous translators.
  • Participating in the discharge planning process, including assisting in the early identification and assessment of clients' needs and liaising with the appropriate community and IH program and services (e.g., mental health, addictions, and/or other IPN or community resources).

Requirements

To be successful in this role, you will require:

  • A degree or diploma in social work, healthcare, or a relevant field (e.g., Registered Nurse or Licensed Practical Nurse).
  • Two years of related experience working in a healthcare environment.
  • Demonstrated knowledge, experience, and understanding of Indigenous people and their culture.
  • Knowledge of federal, provincial, territorial, regional, and Indigenous organizations mandated to provide health services.

Prior experience working with Indigenous populations and proficiency in one or more Indigenous languages are considered assets in this role.

Location and Working Conditions

This casual opportunity is based at Penticton Health Centre in British Columbia, requiring travel as a necessary component of the job. Please note that valid BC Driver's License is required for this position.
