We are seeking a highly motivated and organized individual to fill the role of Care Team Supervisor at VHA Home HealthCare. In this position, you will have the opportunity to work with a diverse team of professionals who share your passion for delivering high-quality client-centered care.

About the Role

The Care Team Supervisor role combines both clinical expertise and operational/systems proficiency to enable positive outcomes for clients and caregivers, service providers, office staff, and funders. The Care Team Supervisor's team includes a mixture of unionized field staff (i.e., PSWs and PS Coaches) at the point of care and office-based staff (i.e., Care Team Scheduling Coordinators).

This role requires strong leadership skills, effective communication, and problem-solving abilities to ensure seamless coordination between team members, clients, and funders. The successful candidate will be accountable for ensuring that clients receive evidence-informed quality care and population-specific quality outcomes, with a focus on client and PSW safety.

Key Responsibilities:

  • Accountable for the overall operations and people management of their geographical team.
  • Ensures that clients receive evidence-informed quality care and population-specific quality outcomes.
  • Supervises a team of scheduling coordinators and responds to escalated client or PSW issues.
Requirements

To be successful in this role, you will require:

  • RN or RPN with CNO registration in good standing.
  • Membership in professional associations an asset (RNAO, RPNAO, CNA Certification).
  • Minimum of 3 years experience in community health preferred.
  • 2 to 3 years of supervisor experience in healthcare strongly preferred.
