Alberta Health Services is seeking an experienced Occupational Therapist I to join our team as a Home Care Specialist. In this role, you will work as a member of an inter-professional healthcare team to assess, plan, and evaluate the care needs of clients and their caregivers/families requiring community-based services.
Key Responsibilities- Assess clients' needs and develop care plans to support their occupational performance and independence
- Provide evidence-informed services, including direct care, consultation, education, and advocacy
- Collaborate with clients, families, and healthcare teams to ensure continuity of care and co-operative partnerships
- Work as a mobile worker, traveling within Rockyview County to provide services
- Operate a laptop computer and mobile phone as part of the job
- Completion of a bachelor's degree in Occupational Therapy from an accredited university program
- Registration with the Alberta College of Occupational Therapists (ACOT)
- Two years of recent home care or acute care experience in the last five years
- Proficiency in computer use, with a focus on critical thinking and problem-solving skills
- Experience working with acute clients and/or long-term clients with a focus on chronic disease management
- Experience in providing case management services
- Experience in pediatric and palliative support and client teaching
