Job Summary

Alberta Health Services is seeking an experienced Occupational Therapist I to join our team as a Home Care Specialist. In this role, you will work as a member of an inter-professional healthcare team to assess, plan, and evaluate the care needs of clients and their caregivers/families requiring community-based services.

Key Responsibilities
  • Assess clients' needs and develop care plans to support their occupational performance and independence
  • Provide evidence-informed services, including direct care, consultation, education, and advocacy
  • Collaborate with clients, families, and healthcare teams to ensure continuity of care and co-operative partnerships
  • Work as a mobile worker, traveling within Rockyview County to provide services
  • Operate a laptop computer and mobile phone as part of the job
Requirements
  • Completion of a bachelor's degree in Occupational Therapy from an accredited university program
  • Registration with the Alberta College of Occupational Therapists (ACOT)
  • Two years of recent home care or acute care experience in the last five years
  • Proficiency in computer use, with a focus on critical thinking and problem-solving skills
Preferred Qualifications
  • Experience working with acute clients and/or long-term clients with a focus on chronic disease management
  • Experience in providing case management services
  • Experience in pediatric and palliative support and client teaching
