Vancouver Coastal Health is seeking a skilled Home Support Supervisor to join their team in North Vancouver, BC.

Job Overview

This leadership role involves supervising a team of home support workers and providing high-quality care to clients. The ideal candidate will have experience working with geriatric clients, adults with chronic and acute illness, and possess excellent leadership and communication skills.

Responsibilities
  • Supervise and manage a team of home support workers to ensure effective delivery of client care.
  • Assess service eligibility and develop individualized care plans for clients.
  • Provide direct care and coordinate services to assist clients in managing their own care and navigating community resources.
Requirements

To be successful in this role, you will need:

  • A graduate degree from a recognized program for Licensed Practical Nurses (LPNs).
  • Current practicing registration as an LPN with the British Columbia College of Nurses and Midwives (BCCNM).
  • At least two years' recent related experience working with geriatric clients, adults with chronic and acute illness, including one year in a supervisory role.
  • A valid BC Class V driver's license and reliable transportation.
