About the Opportunity:

We are seeking a skilled Licensed Practical Nurse to take on a challenging role as a Transition Liaison in our beautiful Vernon, BC location. As a valued member of our team, you will play a crucial part in planning, implementing, and coordinating patient access and care transition activities between our facilities and healthcare services across the continuum of care.

Responsibilities:

• Collaborate with multidisciplinary teams to assess patient needs and eligibility for appropriate services and resources.
    • Work closely with patients, families, and various healthcare teams to provide comprehensive care and support.
    • Utilize your expertise to operate within the full scope of practice outlined by the British Columbia College of Nurses and Midwives (BCCNM), and according to organizational policies, procedures, and unit-specific protocols.

Requirements:

• Graduation from a recognized program for Practical Nurses
    • One year of recent related clinical experience, including a combination of Community and Acute Care nursing experience working with complex patient populations;
    • Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) required;
    • Valid B.C. Driver's License required;
    • Salary: $32.84 to $41.35 per hour.
