Job Summary:

The Community Health Nurse will work independently in the community setting, collaborating with an interprofessional team to manage client caseloads, assess client needs, and develop comprehensive care plans. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a dynamic environment.

Responsibilities:
  1. Establish and maintain therapeutic relationships with clients and their families, ensuring their autonomy and dignity in decision-making and care planning.
  2. Conduct initial and ongoing client assessments, providing individualized care and interventions, and referring clients to other services as needed.
  3. Develop and implement comprehensive care plans, collaborating with the interprofessional team, primary care providers, and community resources.
  4. Provide direct client care, including health counseling, education, and self-management support, to promote optimal health and wellness.
  5. Collaborate with other healthcare professionals to facilitate the flow of information and coordinate care services.
  6. Maintain accurate and up-to-date clinical records, including intake screens, client assessments, and progress notes.
Qualifications:

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting.

Valid BC Driver's license and access to a personal vehicle for business-related purposes.

Demonstrated knowledge, skills, and competence in gerontology and adults living with complex frailty and chronic illnesses. Ability to communicate effectively, both verbally and in writing, and integrate and evaluate pertinent data from multiple sources to problem-solve effectively.

Ability to promote client-focused care, including sensitivity to diverse cultures and preferences, and work effectively in a dynamic environment with changing priorities.

Ability to work independently and as a member of an interprofessional team, and maintain physical ability to perform the duties of the position.
