Nurse Care Coordinator

Job Summary:
The Sioux Lookout Meno Ya WIn Health Centre is seeking a skilled Registered Nurse to coordinate and provide high-quality patient care. As a key member of our interdisciplinary team, you will be responsible for assessing, planning, and implementing interventions to meet the unique needs of our patients.
Responsibilities:
• Collaborate with healthcare professionals to develop and implement individualized care plans
• Conduct thorough assessments to identify patient needs and priorities
• Provide direct patient care, including administering medications and treatments as prescribed
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
• Participate in quality improvement initiatives to enhance patient outcomes and experiences
Requirements:
• Current registration with the College of Nurses of Ontario
• Basic CPR certification (BCLS) required
• Minimum one year of experience in a healthcare setting preferred
• ACLS/PALS/NALS course and/or status preferred
• Good communication and assessment skills
• Ability to work effectively in a fast-paced environment
