Job Summary

We are seeking a highly skilled Registered Practical Nurse (RPN) to join our dynamic team at the Reactivation Care Centre Unit 2 & 3. As a key member of our healthcare team, you will provide compassionate care to older adults with complex needs, including dementia and cognitive impairments.

Key Responsibilities

  • Demonstrate expertise in geriatric nursing practice, prioritizing patient-centered care and promoting independence.
  • Communicate effectively with patients, families, and healthcare teams, ensuring seamless collaboration and coordination of care.
  • Assess patient needs, develop personalized care plans, and implement evidence-based interventions to optimize patient outcomes.
  • Work collaboratively with interdisciplinary teams to achieve optimal patient care and quality improvement initiatives.
  • Participate in ongoing education and professional development to enhance knowledge and skills in geriatric care.

Requirements

  • Current registration with the College of Nurses of Ontario (CNO).
  • Minimum 2 years of recent experience in medical-surgical and/or clinical nursing, preferably working with older adults, dementia, and cognitively impaired populations.
  • Basic Cardiac Life Support (BCLS) certification.
  • Recent health assessment course completion.
  • Bilingualism in French/English an asset.

What We Offer

Competitive salary: $90,000 - $110,000 per year, depending on experience.
