Brain Injury Care Model Implementation Specialist

We are seeking a highly motivated and experienced individual to join our team as a Brain Injury Care Model Implementation Specialist.
About the RoleThis is an exciting opportunity to work with a multidisciplinary team to develop and implement a novel care model for individuals with brain injury. The successful candidate will have expertise in clinical research and experience in supporting and managing teams. They will be responsible for developing a manualized multidisciplinary primary care health team model, creating a clinical implementation protocol, identifying data quality indicators, and overseeing the implementation activities of the clinical team.
The Brain Injury Care Model Implementation Specialist will work closely with the Clinical Leads and Care Pathways Team to ensure that the novel model of care is implemented effectively and efficiently. They will also be responsible for preparing necessary Research Ethics Board documents, enrolling patients into the novel care model, and conducting data analysis to evaluate the clinical and implementation outcomes.
About the SalaryThe salary for this position is estimated to be around $70,000-$90,000 per year, depending on experience.
Key Responsibilities- Develop a manualized multidisciplinary primary care health team model encompassing the 6 lifestyle pillars proposed by Brain Changes Initiative
- Create a clinical implementation protocol that all patients enrolled in this novel model will receive
- Identify and develop mechanisms to collect structure, process, and outcome data quality indicators
- Prepare necessary Research Ethics Board documents to obtain approval for this Quality Improvement project
- Enroll patients into the novel care model, ensuring informed consent and all implementation practices are consistent with the approved quality improvement implementation practices
- Ensure consistent implementation of the clinical, data collection, and research protocols and oversee the implementation activities of the clinical team
- Conduct data analysis to evaluate the clinical and implementation outcomes
- Expertise in clinical research
- PhD in neuroscience/neurorehabilitation preferred
- Self-motivated, detail-oriented, and able to work independently and as a team
- Experience supporting and managing teams
- Desire to create treatment models and influence standard of care
- Comfortable training and supporting health care providers to deliver the novel model of care
- Experience supervising trainees, overseeing reviewing, and providing feedback
- Excellent verbal and written communication skills
This role will be based at the Scarborough East GTA Family Health Team located at 520 Ellesmere Rd, Scarborough ON, with attendance required 2 days per week.
