Baycrest Hospital is seeking a skilled Pharmacist to join its Pharmacy Department. As a key member of the team, you will be responsible for ensuring the safe and effective management of medications for patients across the hospital.

The successful candidate will have a strong background in hospital pharmacy practice, with a focus on patient care and medication optimization. You will work closely with the interprofessional team to provide high-quality care and support to patients, while also contributing to the development and implementation of pharmacy clinical programs.

Key Responsibilities:

  • Participate in relevant rounds and care conferences to ensure safe and appropriate prescribing decisions are made
  • Identify, prevent, and resolve drug-related problems through medication reviews and patient care planning
  • Ensure the seamless continuum of care by conducting medication reconciliation when patients are transferred to or discharged from Baycrest
  • Ensure accurate preparation and dispensing of medications to designated hospital programs
  • Participate in departmental medication management projects and pharmacy clinical programs

Qualifications:

  • Current and valid license to practice as a Pharmacist from the Ontario College of Pharmacists (Part A)
  • Minimum of 2 years of hospital pharmacy experience
  • Hospital Pharmacy Residency from an accredited institution or Doctor of Pharmacy Degree (completed or in progress of completing within the year required)
  • Demonstrated expertise in the provision of pharmaceutical care
  • Self-motivation, excellent communication and organization skills required
