Health Care Aide

Prairie Mountain Health is seeking a skilled Health Care Aide to provide high-quality patient care and support services to our clients. As a key member of our team, you will play a vital role in ensuring the health, safety, and well-being of our clients.
Key Responsibilities- Client Care
- Assist clients with activities of daily living, promoting self-respect, dignity, and independence.
- Follow individualized care plans, providing feedback to improve care and communicating client changes to the team leader.
- Document care according to policy, ensuring accurate and timely recording of client information.
- Personal Care
- Provide or assist clients with personal hygiene, including bathing, grooming, skin care, and oral care.
- Assist clients with elimination, including assistance to the bathroom, bedpan or urinal use, and perineal care.
- Provide or assist clients with mobility, including assistance with feeding, distributing nourishment, and encouraging consumption.
- Observation and Measurements
- Report any changes in client condition to the team leader, updating BM sheets and weighing clients as required.
- Document care on applicable flow sheets, ensuring accurate and timely recording of client information.
- Safety and Infection Control
- Maintain a safe environment, distributing and storing equipment appropriately.
- Follow proper hand hygiene and infection control practices, using personal protective equipment as required.
- Administrative Tasks
- Order and stock unit supplies, including linen and food.
- Encourage client participation in activity programs, organizing and facilitating leisure activities as needed.
- Other Duties
- Demonstrate the safe use of medical equipment applicable to client service areas.
- Perform other duties as assigned by the team leader.
To be successful in this role, you will require:
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong attention to detail and ability to maintain accurate records.
- Ability to lift, move, and transfer clients as required.
- Knowledge of infection control practices and safety procedures.
