Care Coordination Specialist

Vancouver Coastal Health is seeking a highly skilled Coordinator, Patient Care (RN) to join our team in Acute & Ambulatory Care. This is an exciting opportunity to work in a dynamic environment, where you will be responsible for coordinating patient care activities and providing leadership to designated staff.
About the RoleAs a Care Coordination Specialist, you will be responsible for:
- Coordinating patient care activities in a defined area of clinical expertise, including leadership, supervision, and guidance to designated staff.
- Facilitating efficient patient flow and access within and between units/programs/clinics.
- Ensuring high standards of patient care by acting as a clinical resource for staff, supporting staff training and development, and providing leadership to the interdisciplinary team.
- Assisting with administrative duties, staffing, scheduling, and clinical research activities to optimize patient care resources.
- Promoting education and research initiatives.
You will work collaboratively with the interdisciplinary team, leadership, and healthcare professionals/providers to achieve excellence in patient- and family-centered care and quality of life enhancements.

RequirementsTo be successful in this role, you must have:
- A current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Completion of post-basic specialty nursing certificate/program if applicable.
- Three years' recent, related experience in the designated clinical area.
- One year's related administrative/supervisory experience or an equivalent combination of education, training, and experience.
- A valid BC Driver's License and access to a personal vehicle for local area travel may be required.
The salary for this role is estimated to be between $85,000 - $110,000 per annum, depending on qualifications and experience.
