About the Role:
We are seeking a passionate and caring Registered Practical Nurse (RPN) to join our team at Bayshore Healthcare. As a RPN, you will play a vital role in providing high-quality home health care to our clients in the Brockville City service area.
Key Responsibilities:
- Provide clinical care to clients in their homes, ensuring their physical and emotional needs are met.
- Work collaboratively with our interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments of clients' needs and develop effective care plans to meet those needs.
- Provide education and support to clients and their families on health-related topics.
- Participate in ongoing education and professional development to stay current with best practices in home health care.

Requirements:
