Responsibilities:- Conduct regular wellness visits for all residents to assess their health and identify any potential issues.
- Collaborate with the Resident Care Director to develop and implement individualized care plans for each resident.
- Monitor and document resident vital signs, including weight, temperature, and blood pressure.
- Provide education and support to residents and their families on health-related topics.
- Participate in quality improvement initiatives to enhance the overall care experience for our residents.
- Current state license as a professional Registered Nurse (RN).
- Minimum of one (1) year experience in a healthcare setting, preferably in senior care.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic computer skills and proficiency in electronic medical records.
