Job Description

We are seeking a highly skilled Rehabilitation and Community Services Coordinator to join our team at the Thunder Bay Indigenous Friendship Centre. This is an excellent opportunity for a dedicated professional to make a meaningful impact in the lives of Indigenous individuals and families.

The successful candidate will be responsible for coordinating rehabilitation programs, assessing client needs, and developing individualized plans to support their success. They will also work closely with community partners to ensure seamless referrals and services.

This role requires strong interpersonal skills, attention to detail, and the ability to work independently in a fast-paced environment. If you have experience working in social services, a strong understanding of Indigenous cultures, and a passion for making a difference, we encourage you to apply.

Responsibilities
  • Coordinate rehabilitation programs, including intake assessments, goal setting, and progress monitoring
  • Assess client needs and develop individualized plans to support their success
  • Collaborate with community partners to ensure seamless referrals and services
  • Provide support and guidance to clients, including counseling, case management, and advocacy
  • Maintain accurate records and reports, including statistical analysis and program evaluation
  • Contribute to the development and implementation of new programs and services
Requirements
  • Bachelor's degree in Social Work or a related field
  • Minimum 3 years of experience working in social services, preferably in a correctional setting
  • Strong understanding of Indigenous cultures and histories
  • Excellent communication and interpersonal skills
  • Able to work independently in a fast-paced environment
  • Ability to maintain confidentiality and handle sensitive information
