About the Role

The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for delivering high-quality, resident-centered care in our long-term care facility. As a LPN, you will work closely with our interdisciplinary team to assess residents' needs, develop and implement individualized care plans, and provide direct care to residents.

Responsibilities
  • Assess residents' physical and psychological needs, and develop and implement individualized care plans to meet their unique needs.
  • Liaise with physicians, community agencies, families, and other healthcare professionals to ensure seamless care delivery.
  • Administer medications and treatments as prescribed, and monitor residents' responses to care.
  • Participate in interdisciplinary care conferences to review residents' progress and adjust care plans as needed.
  • Support the administrative requirements of the community, including admissions, discharges, and maintaining confidentiality.
  • Actively participate in orientation of new staff, community programs, and educational in-services.
  • Provide coaching and direction to care staff as needed, and use in-the-moment education to improve resident care.
Qualifications and Skills
  • Graduate of an approved program for Practical Nursing and current licensure with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in a long-term care facility an asset.
  • Ability to read, write, and speak English proficiently, and understand verbal and written instructions.
  • Computer skills, including the ability to navigate basic computer programs such as Word, Excel, and Outlook.
  • Demonstrates collaborative and team-working skills.
  • Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand, and walk as required to perform the duties of the position.
