CarePartners is seeking a dedicated and compassionate RN or RPN to join our team as a Community Nurse in Grimsby, Beamsville, and surrounding areas. This part-time role offers flexibility in scheduling and a chance to work in a supportive and collaborative environment focused on patient care.

The Role:
You will develop and maintain effective Care Plans with respect to Community Health Services, working with patients in various settings, including homes, clinics, retirement homes, and schools. You will communicate and liaise with patients, family members, and the interdisciplinary care team, performing other clinical duties as required.
Requirements:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- A strong desire and commitment to patient care
- Excellent interpersonal and communication skills
- A valid driver's license and vehicle for regular travel
Salary: $65,000 - $85,000 per year, depending on experience
