Key Responsibilities

  • Provide comprehensive health assessments and develop individualized plans of care for patients with complex medical needs.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination and communication.
  • Develop and implement symptom management plans, utilizing a broad range of therapeutic modalities to promote comfort and quality of life.
  • Provide education and support to patients and their families, empowering them to make informed decisions about their care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in palliative care.

Requirements

  • Current registration in the Extended Class (NP-Adult) with the College of Nurses of Ontario.
  • Completion of a Master Degree in Nursing.
  • Minimum of 3 years nursing experience in palliative care.
  • Exceptional knowledge in palliative care and pain & symptom management.
  • Formal training and education in Palliative Care required with minimum of LEAP Course completion.
  • Eligibility for cross-appointment to an academic institution preferred.
  • Membership in specialty related organizations NPAO, RNAO.
