About the Role

We are seeking a skilled Clinical Social Worker to join our team at the West Bedford Transitional Care Centre. As a key member of our interdisciplinary care team, you will work collaboratively with healthcare professionals to provide comprehensive psychosocial support to patients and their families.

Your primary responsibilities will include:

  • Conducting bio-psychosocial assessments to identify strengths and challenges that impact optimal health outcomes
  • Determining appropriate therapeutic interventions to address trauma, suicide and crisis, psycho-educational needs, addictions, grief and loss, resource needs, stress management, life altering transitions, and mental health issues
  • Developing and facilitating psycho-educational activities and groups to increase self-care and self-management
  • Working collaboratively with the healthcare team to create a comprehensive plan for risk reduction and timely transition through assessment, planning, intervention, and evaluation

Requirements

  • Bachelor of Social Work degree
  • Current registration with the Provincial Association of Social Workers
  • Minimum of two years clinical experience
  • Excellent communication and interpersonal skills
