Job Description:

  • Provide comprehensive case management work and systems navigation support for young individuals transitioning from homelessness
  • Collaborate with housing providers and internal/external services to support clients in navigating social assistance, education, and employment systems
  • Meticulous documentation and participation in program evaluations to provide holistic support and empower youth to achieve safe, stable housing and address broader social, educational, and health needs

Key Responsibilities:

  • Develop and implement individualized plans to support youth transitioning from homelessness
  • Collaborate with internal and external stakeholders to ensure seamless service delivery
  • Document and analyze data to inform program improvements and optimize outcomes for our clients
  • Foster relationships with community partners to enhance access to resources and services
  • Promote a culture of inclusivity and respect within our organization

Qualifications:

  • College or university degree or diploma in child and youth work or equivalent
  • Minimum two years' experience in a shelter or residential setting
  • Knowledge of adolescent developmental theory and specific issues related to street youth
  • Up-to-date UMAB (Understanding and Managing Aggressive Behavior) Training
  • Excellent verbal and written communication skills
  • Demonstrate leadership skills; ability to work from a trauma-informed, strength-based framework from a DEI perspective
