Job Summary

We are seeking a dedicated and compassionate home health care professional to provide medical support to our clients in their homes. The ideal candidate will have a can-do attitude, excellent interpersonal skills, and the ability to provide personalized care to our clients.

Responsibilities

  • Visit patients in their homes for one-on-one visits to assess their physical, psychosocial, and environmental needs.
  • Complete initial and ongoing assessments to identify the needs of home health patients and chart observations.
  • Administer medication as prescribed by the patient's physician and educate patients and their families on proper home health care strategies.
  • Provide effective communication to patients/clients, their family members, team members, and other healthcare professionals.

Requirements

  • Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing, or vocational nursing.
  • Current license with College of Nurses Ontario and in Good Standing as an RN or RPN.
  • Professional Liability Insurance - Mandatory.
  • Proof of current CPR certification.
  • Compassionate, friendly demeanor.
  • Excellent communication skills.
  • Current driver's license and reliable transportation.
  • Ability to work a flexible schedule and travel locally.
