### Key Responsibilities
* Provide nursing care to patients in a home or community setting
* Enhance quality of life for the patient and family; consult, confer, and collaborate with other healthcare providers
* Demonstrate continuous improvement and evidence-based practice in nursing practice
* Maintain and advance own clinical competence
* Assess, plan, coordinate, organize, implement, evaluate, and transition nursing and other care services for the client
