Job Title: Rehabilitation Service Worker

Join our team at Chezranti Community Centre Ltd as a Rehabilitation Service Worker. In this role, you will be responsible for providing rehabilitation services to clients, including developing and implementing rehabilitation programs, conducting assessments, and providing support and guidance to clients.

Key Responsibilities:
  • Develop and implement rehabilitation programs for clients, including individualized plans and group programs.
  • Conduct assessments and evaluations to determine client needs and goals.
  • Provide support and guidance to clients, including counseling and education.
  • Collaborate with other healthcare professionals and community agencies to ensure comprehensive care.
  • Maintain accurate records and reports, including client files and program statistics.
Requirements:
  • Education: College/CEGEP degree in a related field.
  • Experience: 3 years to less than 5 years of experience in a rehabilitation setting.
  • Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team, and ability to maintain confidentiality.
Work Environment:

The Rehabilitation Service Worker will work in a fast-paced environment, with a large workload and tight deadlines. The ability to work under pressure and maintain attention to detail is essential.
