Supportive Care Team Member

Job Summary:
The Health Care Aide II (Certified) is responsible for providing direct care to residents in our long-term care facilities. This includes assisting with daily living activities, providing emotional support, and collaborating with other healthcare professionals to ensure high-quality care.
Key Responsibilities:
- Provide personal care to residents, including bathing, dressing, and grooming
- Assist with feeding, toileting, and mobility needs
- Monitor residents' vital signs and report any changes to healthcare professionals
- Participate in care planning and implementation
- Collaborate with other members of the healthcare team to ensure seamless care delivery
Education and Experience:
Qualified applicants will possess:
- A diploma in Health Care Aide or equivalent
- 1-2 years of experience in long-term care or chronic care settings
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Current certification in CPR and First Aid
