Job Summary

We are seeking a Licensed Practical Nurse to join our team as a Hospice Care Specialist. In this role, you will provide safe and respectful nursing care to patients, conduct assessments, and communicate with the RN/RPN and other members of the care team as required.

Responsibilities
  • Provide nursing care to patients, including administering medications, monitoring vital signs, and documenting patient information.
  • Conduct head-to-toe assessments and recognize abnormal signs or symptoms, communicating with the RN/RPN as needed.
  • Support and partner with the RN/RPN in the care of patients with complex or unpredictable outcomes.
  • Communicate with physicians and other members of the care team regarding patient care and treatment plans.
  • Document patient care, including assessments, status, progress, treatments, and medications administered.
Requirements
  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience.
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
  • Current basic cardiac life support certification.
