**Key Responsibilities**
- Provide nursing services to clients in their homes, community, or clinical setting
- Work within the context of our client-centered, family-focused approach
- Assist with the triage of client referrals, in accordance with established guidelines, practice standards, and set criteria
- Follow up with clients and/or families, physicians, and other healthcare providers regarding complex medical conditions through client reassessments and the sharing of medical information
- Participate in ongoing planning, development, and evaluation of the Home and Community Care Program
